Random world drop, boe design. Best bet is to watch the auction house, where it is likely to be quite pricey, or hope for a helpful guildie.

I got this in a drop from Utgarde Pinnacle, and gone straight in the AH for a very high price, and considering how much the gem is going for it wouldn't suprise me if I manage to sell this pretty quickly! The gems on my server are going for 100g a pop, so you could probably get a lot more than the Buyout price suggested on here.

I had this drop from a Hardknuckle forager in sholozar.

This is a world drop item, so any mob in [presumably] Northrend can drop it. Mine dropped from a Scarlet Onslaught mob; however, it could drop anywhere, so it's irrelevant.
