I had this achievement bug out on me when defeating creech. I won with different line ups without getting credit. What I did was log out, grab an alt and used this specific line up: Anomalus (2,2,1), Sapphire cub (2,*,*) and Grasping manifestation (*,*,*) and then got the achievement to pop. Just putting it out there in case this happens to anyone else.
