As of 09/11/2024, this achievement is bugged. I hit max rep with the General and received the title, but no achievement. Relogging/logging into another toon did not resolve. Submitting ticket.
EDIT (09/19/24): If you're also having this issue, bug reports people. That's the only advice I got from my ticket. I was the first to report the issue apparently.

as of 2/10/24 this is still bugged. Just hit max rank and no achieve or title has appeared... small indie company aye
I got Mastermind tier friendship with The General today in Dornogal, but the achievement didn't pop and I didn't receive the title. However, when I used the burrow to portal to Azj-Kahet, upon loading in I was granted the title (but still no achievement.) If you're having trouble with it try going to the Weaver's Lair.
