Drops the Netherlord's Accursed Wrathsteed appearance for the warlock class mount after you have already completed the quest to obtain the regular look. It seems as though this isn't a 100% drop chance.
The other appearances are Netherlord's Chaotic Wrathsteed, which is the base appearance obtained by completing the quest The Wrathsteed of Xoroth, and the Netherlord's Brimstone Wrathsteed
