I caught this fishing in the Ocean in Eversong, not a fishing pool. An emote appeared that a chest spawned for me, and it was next to me. Looted it for this sword.

I just fished [url=https://i.imgur.com/3LRSwlx.png]two of these[/url] up in the same cast in the Voidstorm (Slayer's Rise). I have a feeling that this was caused by perception. Even though I believe perception is supposed to only work on resources, I think there is a bug allowing for it to affect these transmog items as well.
