TheWoWDBIf you can down Gruul and Nightbane, getting to this guy in heroic Slave Pens will probably not be a problem

With the help of a couple limited invulnerability potions (to live to sprint past the guys who can see through stealth), and vanish it is possible to stealth to Skarthis as a rogue on heroic mode to pick this quest up and turn it in.
With today's annoucement by Blizzard that there will now be no attunement required to enter Serpentshrine Cavern, it is no longer necessary to do this quest:

However the quest will remain in game; the same post also implies they may add a reward in the future for those who have completed this quest.

Description
Bring me the elemental signets that the cudgel requires! The earthen signet is held by Gruul the Dragonkiller. Gruul can be found inside his lair in the Blade's Edge Mountains. The second signet is the blazing heart of a creature known as Nightbane. This mythical beast is said to reside in the hallowed halls of the prophet, Medivh. Make haste!

Coilfang Reservoir is the name for a naga-dominated compound that lurks under Zangarmarsh's deepest waters. The reservoir, punctured throughout by massive open pipes and pistons, seems big enough to house armies - and, yet, its purpose isn't strictly military.
