TheWoWDBFound in the Deadmines... last part of the instance after you blow down the wall to the area with the ship and the underground lagoon. It'l randomly drop off of any enemy in the area, but with a fairly low drop rate.
You can probably get this is you solo the DM a couple times at a high lvl.
