This dropped off of Darkweaver Syth for us during a Sethekk Halls run. Not sure if it's a random world drop, specific to Auchindoun instances, specific to Sethekk, or truly just part of this bosses loot table, but I figured I'd post where I got it to help everyone else out.
I'd venture to say the drop rate is less than or equal to 5%-ish, or at least it has been for us. We've killed him about 20 times now and just saw it on our last kill for the first time.
