As of today, I have cleared it (Edit: To clarify: Earthcrawl Mines) on each difficulty, 4-10, both solo and in a group, between yesterday and now. Each time there were all revives remaining. No credit given.
After completing Earthcrawl Mines many times, with no success to get it to count with the achievement, it was unlocked for me after I finished Fungal Folly.
