In the teleporter room in Meredil, from left to right, locations of the portals are (and the location of the entrance to the cave/building/etc):

1. Felsoul Hold (39.1, 76.3) (Entrance: 35.4, 82.2) **Internal map coordinates are 53.0, 36.7 **
2. Falanaar (22.0, 29.2) (Entrance: 22.8, 36.2)
3. Moon Guard Stronghold (30.8, 10.9) (Entrance to area: 26.3, 25.9) **see Note 1**
4. Lunastre Estate (43.6, 79.1)
5. Ruins of Elune'eth (36.4, 46.8) (directly above that room)
6. The Waning Crescent (47.6, 81.6) **See Enixxe's comment below**
7. Sanctum of Order (43.4, 60.7) (Entrance: 46.0, 64.6)
8. Tel'anor (42.2, 35.4)
9. Twilight Vineyards (64.1, 60,8) (Gotten after completing the vineyards chain quest 'Vengeance for Margaux')
10. (New!) Astravar Harbor (54.4, 69.5) Cannot get this portal without completing quest lines. This portal is right next to Oculeth.

Note #1: You can cheese getting to this one by going to the Obsidian Overlook Flight Path in Highmountain, and flying/floating/hang-gliding off the edge, headed west. You can also pick up the +100 max Ancient Mana item, Infinite Stone, with this trick as well.
