Around the Zaqali Caldera region, you'll find [object=386083]s that give you an 1 hour debuff. Find 3 orbs before the debuff falls to make the chest visible to you. When the chest is looted, all the orbs disappear and don't spawn again for several hours. The cooldown for orbs respawning has been observed to be 1 hour.

Has anyone had the problem with the bloody body and it not being there, despite a completed campaign or did I miss something?

The last "treasure" I needed for this achievement was Bloody Body, which just never appeared for me despite completing the campaign. As it turns out, I had to advance a nearby side questline at the Dragonscale Camp, involving Rupert the Gentleman Elemental and the Buried Vaults subzone.
Only when I received the quest A Tower Yonder did the body of Sheridon Hastle appear, allowing me to interact with it and complete the achievement.
