Gnomish Death Ray has ridiculous damage. I have seen level 39 gnomes crit with this for thousands. Rogues especially can combo this with gouge (if you have improved gouge). This way, the target is safely incapacitated while you charge up your huge burst of damage.

A tactical way for a warrior could be that you use intimidating shout and then the trinket.
On my 60 mage i can use this and crit for 3k and take zero mana from the drain, because the damage is physical damage and is absorbed by mana shield. Good combo to hit someone in a frost nova.
