No, you aren't crazy. This was just added to the game in patch 7.2.5. You get it for finishing a heroic run or higher of upper karazhan (you won't get it if you do lower kara).
Achievement
Heroic: Return to Karazhan
Defeat Viz'aduum the Watcher in Return to Karazhan on Heroic difficulty or higher.

Thanks! Glad to see this. I just got this on a random heroic run and thought something had to be wrong since I've completed upper kara many times already or that possibly some of my achievements had somehow gone missing. Glad to see I'm not (completely) crazy! :P
I have achievement for completing dungeon in Mythic difficulty, but don't have this one. Is it bug?
