This achievement is done at the first boss. He will spawn 3 adds at the start and when these 3 dies, he will spawn 3 new after a while. So if you would accidentaly kill one of the adds more will spawn :)
The boss will do a cone looking spell, easily seen on the ground. and it takes out about 45% hp on the adds.
So get the adds low then place them in the cone while hes loading the spell. Wait till u have killed 3 adds this way, then kill the boss.

While it does not word it properly in the achievement you need to kill the mob before it will award the achievement. My group ended up killing roughly 10 of the adds thinking it was bugged but once we killed him the achievement popped.
Just a bit of further clarification. As already mentioned, this takes place at the first boss. That doesn't mean the first boss inside the cave, but the at the very first encounter BEFORE you get on the boat.
