To get this quest you need to get prestige level 1 AND then to get 50 level again.
The reward for level 50(after you are prestige level1) is a quest item that starts this quest :

You've met the requirements when you reach PVP Prestige Level 2. You don't have to do rated.

When you reach Prestige level 1 - 49, you'll see the reward for reaching level 50 is the Honourable Medallion. It goes into your bag autumatically. Clicking on it opens the quest to see your faction leader. If you threw out the Medalion contact a GM.

Prestige Level 1-50 is the same as Prestige Level 2 - 0. You'll see the flagged icon to the left of your portrait has changed.

After you complete the quest, go to your class hall to pick up your new weapon appearance. Change specs to pick up the weapon appearance for your off-specs. If your weapon(s) are transmogged you won't see the new appearance but your weapon(s) will look different on your character panel.

Description

We will never be dominated so long as you stand at the front of our lines. Together we will slaughter anyone who stands in our way. You have sacrificed as much as any in the name of the Horde. I will not let your dedication go unnoticed. Face your fellow champions, {name}. Face them, and be recognized.
