In the Trial of the Grand Crusader, reach a Tribute Chest with at least 45 attempts remaining in 10-player mode.

To obtain this achievement, you must complete the 10-player version of Call of the Grand Crusade (the new raid dungeon in patch 3.2) while wiping only five times.
We just got this achievement and only got the normal loot from the tribute chest along with 4 Trophies. Most likely the cloak only drops with Tribute to Insanity chest
