Doesn't Go to Eleven is earned inside Pit of Saron, a dungeon. The entrance is marked below.

Got this through sheer luck with Frost Res Aura up. A full resist will prevent the debuff froms stacking further.
Edit: To whoever rated this down, I don't mean to say that you're guaranteed to get the achievement for having frost resistance. It may sometimes prevent one application of the debuff, causing it to fall off entirely... but you need to be lucky, or stack more frost res perhaps, to get the achievement with this alone. And if you find this incorrect, at least take the time to reply and report your findings.
This achievement isn't loud enough.
