You can do this one totally geared. Do note there are a couple other easy achievements you can do as well (kiss Sara at some point and kill 9 of those adds Sara spawns really fast). The only real killer to this fight is looking at the skulls too long in phase 2 (sanity loss) or not killing the 9 yellow adds and brain fast enough.
