To clarify. Either the whole raid will get this achievement or no one will.
The achievement is obtained by making sure that every portal is taken by someone every time portals spawn.
When a portal spawns, it will despawn when someone enters it or will despawn on its own after a few seconds. You just need to make sure that none despawn on their own. It does not matter who takes the portals, just that every single portal is taken every single time.
The achievement text is pretty misleading as it makes it seem as though you get it on a personal level by taking a portal each portal phase.
Just completed this achievement without using portals. I healed her as fast as possible before any portals could open! :D
