An achievement raid where I was in got this achievement by accident. One of us pulled Yogg too soon so that the rest of us was locked outside the room. Even after the player who had pulled the boss died the combat didn't reset - and the faceless adds still kept spawning. They simply ran through the walls where our tanks picked them up. We just waited until we had enough adds before aoeing them down. Of course we died after getting the achievement because we were still locked out, but it was quite amusing because the adds literally came out of the walls.
They mostly come at night... mostly.
