I found one after being stuck on 44/45 and going round all the listed ones again without any luck. There is one called "Warp Cache" in a small cave at 58.65, 33.76 that's floating in a big portal at head height.

I had the same issue with being stuck at 41/45 with HandyNotes. I found some chests at locations not previously visited through HandyNotes that dinged the achievement for me:

/way 71.5, 49.8 Jandvik (Top Floor)
/way 32.3, 77.1 Felsoul Hold (Inside the Legion Ship)
/way 81.9, 57.5 Underwater Cave
/way 68.6, 47.2 Jandvik

Hope this helps some of you :)
