If you are after this I have some very helpful tips!
1 Don't farm this with warmode on because the task won't show up if you are alone doing the time rift. You need to pump the total score up to make the portal/task appear so you need random people to be doing the time rift with you. Camping the spot where the portal spawns with warmode off is what finally got me the quest item.
2 It's very easy to see which portal the npc will come out of because theres a big circle of ice underneath it. Be quick to tag the mob!
3 The mob will be named after a random nearby player so targetting macros won't work.
4 I got it during a rift where scourge was secondary and not the main invasion :)
