If the quest Holding the Spire is active, you must complete it first to see Khazaduum.
The best way to go to Khazaduum is to click on the navigation console on the "Vindikaar" and get teleported to the Destiny Point. Follow the way down and you can see the entrance to Nath'raxas Spire from a distance. Go in and then go straight ahead until you reach Khazaduum.

The game data tunes Khazaduum for level 45, Legion content.
The map marks it with vignette 2,197, which the client hides once hidden tracking quest 48561 is complete. In game, /run print(C_QuestLog.IsQuestFlaggedCompleted(48561)) prints true when that quest is flagged for your character.
