Hang a slight left at The Defiled Path after leaving Illidan at The Veiled Den. Head uphill to the Felsiege Rise bridge on the right, passed the two huge fel cannons. Cross the bridge and enter the Den of Fiends.
You need to pass two bridge and get in to the cave. Just need to kill mobs to get to 100%.
