In regards to the item being unlootable by all players on the quest, here's what a GM told me, and I have verified for real and for certain that it works, 100%:
Normally when a player dies during a boss fight, they need to wait until the boss is dead before releasing their spirit, or else they will not get the quest item, assuming their group downs the boss while spirit is still outside the instance.
The mechanics of this fight, however, don't register Malygos' death until AFTER Alexstrasza does her thing. If you have died during the fight and release immediately after Malygos is dead, you will notget the quest item. Instead, do not release until Alexstrasza has finished her spiel.
The GM handled my issue and got me my quest item since they verified I was there and had the quest yadda, yadda, but I passed the info on to my friends. We had two people purposely die and release at different times. The one who released before Alex came didn't get the item, the one who stayed through Alex before releasing did. This works for both normal and heroic versions.
Only the person who loots the iris from Sapphiron in normal can get this quest and complete it.

Description
Now that you have access to the focusing iris it's time to put that key to use. As much as I would like to see Malygos brought back into the fold, that's just not going to happen at this point. With everything that's at stake, it is the judgment of the other Dragon Aspects that the Spell-Weaver needs to be put down. <Krasus says this last with a great deal of trepidation.> {name}, travel to the Eye of Eternity, confront Malygos and return with the Heart of Magic.
