The fortune you receive is random and doesn't depend on your answers. The buff, on the other hand, depends on your answers.
Don't expect fantastic loot when you take this quest. You'll most likely get low level food, a potion or two and maybe a coarse stone or comparable items. The main reward for this is rep from the Darkmoon Faire, if you're after a high rep with them.
The loot table for the mysterious lockbox shows that greens and even one blue have come from it, but having done several of the "your fortune awaits you..." quests, I have yet to see anything other than food, potions and perhaps a tradeskill item every so often.
Just don't expect epic loots. :)

Description
"Your fortune awaits you outside Palemane Rock." The fortune Sayge handed you feels slightly warm to the touch. Grasping it firmly, you see an image of the Palemane Rock of Mulgore. Focusing closer in on the image, you see a tree stump that sits close to the cave entrance.
