For me it dropped from mirror at /way 58.8, 67.8 Inside House with Stonevigil Citizen (group 2 of mirrors). Probably can drop from every broken mirror.
Drop is probably a very low chance, same for transmog pieces. You can repair each mirror once per day and every day group of 3 mirrors is active. To repair mirror you need to buy [item=181363] from Simone at Sinfall (from main hall head towards lower level teleport, before mirror turn left). Each repair kit costs 20 [currency=1820]
