If you are a higher level going for Glory of the Cataclysm Hero and you DPS him down right away YOU WILL NOT get this achievement. Let him hit you with one stack of Nauseated first and then kill him.
Edit* You can also eat the bad food to get a stack and then proceed to kill him.

The encounter was bugged for me, Cookie just sat in his pot and did nothing. I still got the achievement when I killed him, so I didn't have to worry about getting one stack of Nauseated.
Don't click the green-glowing food, only the gold-glowing one and you'll get this achievement.
