So had 22 followers at 650 ore more and no achievement.
you need to make sure you have 20 active followers, then you need to upgrade anyone of them with any upgrade.
This triggers the "raise " part of the achievement it seems.of course you could just have 19 active and raise the 20th to 650, but seems most people are like me deactivating capped followers to levels others.
so make sure all 20 are active and then drop any uprgade on any of them.

Anyone who unlocks this will most likely be the next raid boss.
Looks like it might be bugged... got my 20th to 650+ today and I didn't get the achievement. None are on missions or working or inactive.
