This is the reward for killing Algalon in Ulduar frst on your server. Algalon can be unlocked by defeating the 4 watchers with their hard mode active, and has been pre-emptiverly nicknamed 'Algalon the Raid Destroyer' by blue posters (he feeds on your tears!).

At the end of WotLK I used this achievement to try to get into a pug bound for Ulduar hard modes.
I got rejected, they wanted a *real* achievement.
According to the guild Wraith (EU server Ysondre) as reported by MMO Champion.com, Algalon despawns after 1 hour of attempts, similar to Vaelastrasz did in Blackwing Lair.
To clarify: He can only been attempted 1 hour per raid week.
