Having thought I completed this, I couldnt figure out why "Old Soldier or Most Loyal" was still greyed out. Turns out I stopped about midway through the questline leading up to choosing Sylvanas or Saurfang. The quests themselves showed up in my Quest Log right up top under "War Campaign" with a big red banner. Finishing the entire questline will complete this for you.

If you are trying to do this achievement out of order and are lost in the starting the quest chain. I was able to find Lor'themar at New Home in Najatar and was able to pick up the Visions of Danger quest.
IF you're like me and you're looking to do this on alts or characters that didn't get much use during BFA, yes, you have to do the entire war campaign. This is around 101 quests in total. The title is not account wide.
