After defeating [npc=236948] in the [zone=15991] delve on Tier ?, Brann Bronzebeard in Dornogal with have an additional dialogue option allowing you to change his hat.
You'll be able to cycle back and forth between his regular hat and several randomized hats as often as you'd like, and can earn the achievement in a few seconds.

Ask Brann to change his hat ten times after recovering it from the Underpin during the War Within Season 2.

For complete clarity you only need to beat the Underpin on ? difficulty to be able to do this achievement. You do not need to beat him on Double ? difficulty as well.
In fact, you must do it on single-?, as double-? doesn't unlock this.
