The Maddening Deep is located in the south part of Azj-Kahet.
Fly to cords 67 83 and stand in the pool until you reach 100 stacks.
Boom! You got the achievement.

To get stacks, I clicked on the "Black Blood Extractors" which are scattered around the area. Look for the red containers.
Standing in the pools would not give me any stacks, for whatever reason.
