I have completed The Upper Reaches twice today (layer 1) and have not received the achievement and it is not counting towards the Touring Torghast achievement. I have raised a support request with Blizzard regarding this, possibly an issue since the 9.1 update came out a few days ago.

Support ended up giving me the achievement manually, worth contacting until you receive a human response if you are having the same issue.
