Be careful when you are at the last boss, I think we killed the mobs that spawn at the last boss too fast. So Mutanus the Devourer never spawned.
I could be wrong though.

Bugged out for us, too. We walked out, reset all instances, redid it and let the slimes and snakes get to Naralax before killing them and Mutanus then spawned.
