To spawn him you must loot a letter on the ground and take it to the mailbox near the gate to slaughter square.

If you click on the mailbox before finding the letter, just return to the mailbox after you find it. When I approached, he did not spawn right away, but when I turned around to backtrack through the dungeon he spawned a few yards in front of it.
