TheWoWDBDig up and kill the tiny worms until the big worm spawns. Killing it gives you the key for the chest. The chest has to be dug up in front of the small pond to the right of the first pond you find.

There are reddish-brown dust clouds throughout the cave. If you use sniff on the cloud, Myritt says, "Phew, that's worm stink!"
Use Burrow and Dig on top of the dust and a worm will spawn. Not 100% sure of the trigger for [npc=204434], but he spawned after I killed 3 smaller worms and dropped the [item=205941].
Diganthor can respawn, so just be aware of that.
just to add the chest is not part of the dust clouds use the sniff ability around the tunnels to find where the chest is it was an invisible dig spot until it was uncovered by the sniff ability
