Val'anyr, Hammer of Ancient Kings

I don't know about you guys, but I'm doing this quest for the 22g reward.

Here is how I completed this at 120. Note that the quest says it has to be done on heroic however there now only 1 Ulduar setting as far as I know, regular 10 man. I did this with no keepers up/helping me. Some of these steps people probably know as they relate to how to kill Yogg but I'll repeat anyway.

1 Kill the adds near Sara and Yogg will appear when they die on top of her and her health gets to 0.

2 You should be caught in one of Yogg's tentacles after this happens, kill it to free yourself

3 You will see portals open into Yogg-saron's mind, go inside and kill the illusions and the chamber to kill Yogg's brain will be open.

4 Kill Yogg's brain and come back out to the main room. He was at about 30% after this for me.

5 Adds will come out and be attacking you. DO NOT use anything with cleave or you will kill Yogg. Wait for him to cast defeaning roar and spam the quest button to throw the fragments into his maw. I didnt even bother killing the adds. It took less then a minute for him to cast this.

6 After throwing the fragments in you will see a debuff icon under his health bar. If you mouse over it you will see something about the fragments being inside Yogg. This is how you know it has succeeded. There is also a short cooldown of 20 seconds on the quest use icon.

7 The quest will not show as completed at this point, but as long as he has the Val'anyr fragment debuff on him you are ok to kill Yogg. Kill Yogg.

8 Open the loot chest and you will loot the hammer of ancient kings item. It is not looted off Yogg but inside the loot chest.

9 Take the teleport back to the antechamber, turn in the quest at the room behind the assembly of Iron. Done.

I've done this quest and I was a little aprehensive while fightning Yogg, afraid I might screw up the throwing the fragment part.

It's simpler than I thought. The item that you need to throw has a 20 second cooldown, in case some are wondering if you "throw it once and lose it". The item can ONLY be used while Yogg is casting deafening roar, which makes this a lot easier. If you try to use it any other time during the encounter, you will get an error message "No valid targets". So, if the item is used and on a cooldown, your throwing was succesful and all you have to do is kill Yogg.

Also, it is worth mentioning that you do not need to face Yogg to throw the item. You can have your back to him to avoid sanity loss and still throw it.

After Yogg is dead, loot your quest item and turn it in at the room after Iron Council.
