TheWoWDBHad a weird bug where this helm kept breaking - it would show up red in the little broken armor indicator that pops up. I repaired multiple times and it seemed to fix it very temporarily (it would get broken again right away, despite durability being max).
Just taking the helm off and putting it back on seems to have fixed it for me, for now.
