0. Achievement tracker should probably turn from Red to White when criteria met, but I didn't need it as it was obvious when the hourglass shattered before boss death.

1. Mythic Difficulty.

2. Pick up the Infused Hourglass on the right hand side of the room before going down the ramp near the boss.

3. Begin fight, without killing boss.

4. Wait for Eon Shatter circle to appear, then drop hourglass in it.

5. Hourglass Shatters.

6. Kill boss.

7. ?

8. Profit. Achievement Completed.

Just before the boss room on the right hand side you'll find the Infused Hourglass. Click on it to pick it up and bring it to the boss room.

While holding it you cannot cast any spells, and your extra action button will let you put it on the ground. Putting it down puts a timer over it's head. We never let this timer finish but we presume it would blow up or be lost so we just juggled it. You can technically have one person just hold it the whole time and lose some damage, or you can put it down, get some damage, then pick it back up. If you're going to do this then you should note that picking it up has a 2 second cast time so don't cut it close.

As for the achievement, the person holding it needs to be hit by the Eon Shatter, which is the large circle you're supposed to dodge out of when the boss jumps across the room. Given how much damage it does, you should either have your tank carry it and just eat the Eon Shatter, or drop it and run out before the Eon Shatter kills you, but with enough time that it's timer doesn't run out. You may also be able to use an immunity like bubble and then pick it up, but we never tried.

Pick up the Infused Hourglass and drop it in one of the Eon Shatter cirles. When done, you will see shattered glass for about 2 seconds until it disappears. This means you can kill the boss.

For us it did not work to hold the Hourglass and stand in the ability. It just did lots of dmg and nothing happened to the hourglass so we were not sure if we can kill the boss or not. Just drop it and it will work :)
