As mentioned before here, you have to kill the NPC that spawns after 4 minutes.
You can leave the worldsoul memory by right clicking Worldsoul Memory buff after you kill him and get credit.If you leave before killing this unit, even with score, you will not gain credit on achievement.
If you're going to burn 1x Radiant Echoes to chain these back to back for achievement credit, do not do Descendants of Distant Waters on the north side of Hollowfall. It's troll and doesn't always register that you've killed the 4m NPC.
Reign of the Old Gods on the south side works consistently.
