Slimy Yet Satisfying

In the back right hand side of the room as you enter you'll see the [npc=225880] hanging out near the edge of the platform. Here is a picture of the little homie. All you have to do is ensure he lives through the fight. He is a friendly target but isn't able to be healed and his health doesn't appear relevant because for us he was oneshot by anything that happened to him.

He can be herded similar to the variety of world quests we've had since Legion as well as the doe from the Smolderon achievement, but it's not nearly as consistent, and he also runs around on his own a bit as well. It can be difficult to do so, but your best bet it to pull boss away from him on pull and then have someone assigned to herd him away from the boss throughout P1.

If you have the gear/DPS when doing this, I'd go 0 or 1 healers and just zerg the boss to skip the intermission phase as this is really the only time the grub is going to die. If you can't meet the DPS check, then you'll want to position your raid in such a way that the boss' charges won't hit him when they spawn. For us, being either 90 degrees to the side of the grub, or the furthest possible point away from it both seemed to work.

If the grub does die it doesn't happen instantly and isn't super obvious (no death animation, no body) he simply despawns and so if you want to know instantly when he dies I'd recommend using the Instance Achievement Tracker addon which will let you know in raid warning immediately. It's also a fantastic addon for raid and dungeon achievements (dungeon achievements >:( ) in general.

As long as the grub is alive you don't need to do anything else, simply kill the boss.
