I wonder why Kael's Vial Remnant doesn't give this achievement too. You could have killed Kael'thas without getting any loot but not Lady Vashj yet.

This isn't true at all. I've been running this on all of my characters, just run on through to the last boss if you want.
Loot can't be tracked in case you delete the quest and such, which is why when achieves came out most people didn't have old dungeon ones unless there was a quest that included the final boss for it, or had an item that dropped off it (as such)
