Tooth picks can sometimes be gotten from saying "trick or treat" to an innkeeper during Hallow's End. You'll get a stack of ten tooth picks (soulbound). Using one will complete the achievement and give you a 1 minute "Sparkling Smile" buff. The buff makes your teeth sparkle (like in the toothpaste commercials), even through your helmet.
EDIT: Tooth picks are now purchased from [npc=53728] and [npc=53757] for 2 [item=33226]s, or looted from [item=37586], which is received from the candy buckets in inns.
