Description is straight forward. Capture a battle pet when it's below 5% hp. I got it from a small frog in Northern Barrens at the forgotten pools. The frog had 253 hp, I caught it with 11 hp remaining.
Got achievement and a rare frog. Pretty good deal!
Just heard from a friend giving me this tip: He used his Terrible Turnip and used [petability=826]. no need to fight the undead, go for any pet you want.

This achievement is easiest to do against an undead pet imo. You can whack away without the frustration of accidentally killing them, since their passive brings them back to life after death for 1 round.
[npc=61905] are level 1-2 and can be found around the Ruins of Lordaeron (upper floor of the Undercity). Very accessible for horde.
If you can get a [npc=63365] for [petability=826]. You'll be able to do this achievement a lot easier.
