The NPC 'Murloc You' that offered this quest spawned for me right after I completed a random 'Kill as many murlocs as you can' stage, so my guess it that it's an RNG spawn after completing any stage of a murloc time rift.
You get this item from a murloc that follows you around during the murloc invasion, and am not sure what causes him to spawn but he spawn after i killed the murloc version of Garrosh Hellscream (Grrrmrrsh Hrrnglscrrrm).
