TheWoWDBI can't figure out why I'm not finding this item, even after watching a video. Does it just not spawn every time? Is it random?

Been doing time rifts for months now, and still have never seen this quest item... all the rest of the items for this achievement are done. This is such a stupid pain in the ass.
Finally got one. Went on a populated realm (Ragnaros EU). Got the "Kill the [npc=205682]" as 2nd task, and there it was, didn't have time to screenshot but go the Disc on the ground.
