If you happen to forget to pickup the item and left the instance after finishing you can run to the entrance of the instance and walk in to still pick it up. :)
This item can only be looted from the heroic version of the instance and all dungeon quests from the Tavern/Inn now require you to kill the last boss in each instance also.
If you are as geared as you can be and/or don't really want to run heroics, you can also complete the dungeon on normal and still get credit for the boss kill required. You then just extend the heroic lockout and run back in on heroic to collect the item.
