There appears to be a bug. I've completed the requirements but the quest giver is no longer at my inn, or anywhere in my garrison for that matter. The golden "?" is pointing me directly to the fountain in the middle of my garrison, but she isn't there, stealthed or otherwise.
This item can only be looted from the heroic version of the instance and all dungeon quests from the Tavern/Inn now require you to kill the last boss in each instance also.
If you are as geared as you can be and/or don't really want to run heroics, you can also complete the dungeon on normal and still get credit for the boss kill required. You then just extend the heroic lockout and run back in on heroic to collect the item.
