Run a T11 delve and complete it and you will receive the [Cracked Keystone] item which will give this quest. You then do a Keystone Dungeon of any level, and after that you can turn this quest in (you complete it from your quest log, there is no person to turn it in to).
This is once a character, once a season. However, it does bug out sometimes and can be done again at the end of a season.

Description
Fragmentary images of heroic deeds flash though your mind as you touch the stone, but they fade as fast as they arrive. Perhaps performing a suitably impressive task will stabilize the stone's magic once more?

I had this quest completed in my quest-log for a while, and couldn't figure out where to turn it in! - thanx for explaining it should be completed from the log - I had to untrack it and track it back for me to complete it!
